No Surprises Act – Good Faith Estimate

At Nurture Maternal Health, we are committed to transparency and ensuring you understand the costs associated with your care. The No Surprises Act, a federal law, provides you the right to receive a Good Faith Estimate of expected charges for services prior to scheduling care.
Your Rights Under the No Surprises Act
You have the right to request and receive a Good Faith Estimate of the expected costs of your care before beginning services.
This estimate includes the expected charges for therapy sessions and any related services provided by our practice.
A Good Faith Estimate is not a contract, and the actual number of sessions or costs may vary depending on your progress, needs, and goals during therapy.
How Nurture Maternal Health Ensures Transparency
Ohio state licensing rules require us to provide you with the actual cost per session in a written informed consent form. This will include all fees associated with services, which you must agree to prior to beginning therapy.
You will not be billed for any services until you have signed the informed consent form agreeing to these charges.
Understanding Therapy Costs
It is often difficult to estimate the total number of sessions you may require, as this depends on the nature of your concerns and the progress made during therapy. Your therapist will discuss a plan of care during your initial sessions to help set expectations.
You have the right to discontinue services at any time. Termination of services will also follow the terms outlined in the informed consent form.
Disputing Charges
If the actual charges substantially exceed the Good Faith Estimate (by $400 or more), you have the right to initiate a dispute process.
While this is highly unlikely to occur at Nurture Maternal Health, we are committed to addressing any concerns promptly and in compliance with state and federal regulations.
Fee Changes
Any changes to our fees will require an updated informed consent form. You will be informed of and must agree to any fee changes before they take effect. If no changes are made, fees will remain consistent for at least 12 months.
